29 CFR  1910.219(c)(4)(ii): The unused keyway(s) of projecting shaft end(s) were not filled up or covered:  a) Planner Mill; On or about October 15, 2020 and at times prior to the employer exposed employees to amputation hazards in that the employer required employees to work with and around unguarded shafts with unused keyways at the front side of the planner machine next to the adjustment wheel exposing the employees to amputation hazards.

29 CFR  1910.265(c)(18)(i):Pulley(s), sprocket(s), sheave(s), drum(s) and block(s) of conveyor(s) were not arranged to prevent the possibility of injury due to hands or parts of clothing being caught between the belt(s) and pulley(s) or chain(s) and sprocket(s):  a)Planner Mill; On or about October 15, 2020 and at times prior to the employer exposed employees to amputation hazards in that the employer allowed employees to work with and around unguarded belts that runs a motor turning the conveyer at the planner mill exposing the employees to amputation hazards.    b) Planner Mill, Infeed belt; On or about October 15, 2020 and at times prior to the employer exposed employees to amputation hazards in that the employer allowed employees to work with and around unguarded chains and sprockets at the infeed belt to the planner mill exposing the employees to amputation hazards.

29 CFR  1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :  a) Stamper; On November 17, 2020 and at times prior to the employer exposed employees to a noise dose of 129% (equivalent to an 8 hour time weighted average TWA of 91.8) which is approximately 1.29 times the permissible exposure level of 100% dose (equivalent to an 8 hour TWA of 90dBA).  Results are based on a 440 minute sampling period conducted.  Exposure calculation include a zero increment for the 40 minute period not sampled.  The employer did not conduct annual audiometric testing to determine the effectiveness of controls put in place by the employer.    b) Grader; On November 17, 2020 and at times prior to the employer exposed employees to a noise dose of 170% (equivalent to an 8 hour time weighted average TWA of 93.8) which is approximately 1.7 times the permissible exposure level of 100% dose (equivalent to an 8 hour TWA of 90dBA).  Results are based on a 434 minute sampling period conducted.  Exposure calculation include a zero increment for the 46 minute period not sampled.  The employer did not conduct annual audiometric testing to determine the effectiveness of controls put in place by the employer.    c) Maintenance; On November 17, 2020 and at times prior to the employer exposed employees to a noise dose of 165% (equivalent to an 8 hour time weighted average TWA of 94.7) which is approximately 1.65 times the permissible exposure level of 100% dose (equivalent to an 8 hour TWA of 90dBA).  Results are based on a 411 minute sampling period conducted.  Exposure calculation include a zero increment for the 69 minute period not sampled.  The employer did not conduct annual audiometric testing to determine the effectiveness of controls put in place by the employer.    Garrison Brothers Lumber Company was previously cited for a violation of this Occupational Safety and Health Standard 1910.95(g)(6), which was contained in OSHA inspection number 1242279, citation number 1, item number 1 and was affirmed as a final order on September 20, 2017, with respect to a workplace located at 72 US 82 in Eufaula, AL.
