29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  Front left side of residential home 5615 Tranquil Dawn:  On or about October 13, 2020, and at times prior thereto, workers were exposed to a struck-by hazard while operating a pneumatic nail gun without wearing eye protection.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  Front left side of residential home: On or about October 13, 2020, and at times prior thereto, workers with no fall protection were exposed to fall hazards when working from a pump jack scaffold that was on an unstable foundation.   The Ruben Perez Framing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(c)(2), which was contained in OSHA inspection number 1204176 , citation number 1, item number 2, and was affirmed as a final order on March 10, 2017, with respect to a workplace located at 28411 Vario Ln. San Antonio, TX 78260.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  Front left side of residential home: On or about October 13, 2020, and at times prior thereto, workers were exposed to fall hazards while working from a pump jack scaffold without using a fall arrest system.    The Ruben Perez Framing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1204176 , citation number 1, item number 3, and was affirmed as a final order on March 10, 2017, with respect to a workplace located at 28411 Vario Ln. San Antonio, TX 78260.
