29 CFR  1926.501(b)(11):"Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  a.)  On or about October 14, 2020 employees were exposed to an 8 foot 7 inch, a 6 foot 8 inch and an 8 foot 6 inch fall hazard while performing roofing activities on a commercial rooftop with no conventional fall protection system in place.

29 CFR  1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed.   a.)  On or about October 14th, 2020 an employee was observed using an A-frame ladder supported against an exterior wall to gain access to the upper landing surface of a commercial roof that was not being used for the purpose it was designed.

29 CFR  1926.502(a)(2):Employers shall provide and install all fall protection systems required by this subpart for an employee, and shall comply with all other pertinent requirements of this subpart before that employee begins the work that necessitates the fall protection.  a.) The employer does not protect employees from falls by installing a complete fall protection system before employees begin work on a commercial rooftop thus exposing employees to an 8 foot 7 inch, a 6 foot 8 inch and an 8 foot 6 inch fall hazard.  The most recent incident of such failure occurred on or about  October 14, 2020.  Almaraz Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR1292.502(a)(2), which was contained in OSHA inspection number 1332292, citation number 1, item number 2 and was affirmed as a final order on 01/11/2019 , with respect to a workplace located at 2625 Virgie Ct. Loveland, OH 45140.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  a.)  The employer does not does provide training to his employees in order to recognize fall hazards such as, but not limited to, an 8 foot 7 inch, a 6 foot 8 inch and an 8 foot 6 inch fall hazard while performing roofing work on pitched roofs nor are they trained on when to utilize a fall protection system.  The most recent incident of such failure occurred on or about October 14, 2020.  Almaraz Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR1292.503(a)(1), which was contained in OSHA inspection number 1467784, citation number 1, item number 2 and was affirmed as a final order on 08/19/2020 , with respect to a workplace located at 5699 Brooks Run Liberty Township, OH 45044.
