29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a)  On October 13, 2020, employees performing carpentry work were exposed to fall hazards from the roof deck of a three story building.  b) On October 14, 2020, employees performing carpentry work were exposed to fall hazards from unprotected openings on the 3rd floor deck of the building.  Old Timber Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1) which was contained in OSHA inspection number 1425595, citation 1, item 1, and was affirmed final order on November 19, 2019, with respect to a workplace located at 1452 N Leavitt St, Chicago, IL 60622.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
