29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary to enable each employee to recognize hazards related to ladders and stairways, and the procedures to be followed to minimize these hazards: East side of the hotel: On or about October 14, 2020, and at times prior thereto, employees were exposed to a fall hazard while using a 24 ft. Fiberglass Werner Extension Ladder, 5 ft. Fiberglass Step Ladder, and a 6 ft. Husky Fiberglass Step Ladder to install LED lights, without proper training on ladder use and placement.
