29 CFR 1910.23(b)(9):(9) Ladders was not inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury: Main Exchange: Employer had not ensured that inspections of the 12 foot and 10 foot ladders used in the facility during sign hanging and maintenance operations were conducted before initial use in each work shift. Employees were exposed to fall hazards.

29 CFR 1910.1200(h)(1):Employees were not provided an effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Edwards AFB Main Exchange: An employee used Zep Commercial Acidic Toilet Bowl Cleaner and First Street Super-Acrylic Floor Finish to clean restrooms and the retail store. The employer had not provided chemical specific safety and health information or training on the chemicals. The employee was exposed to dermal irritation.
