29 CFR 1926.453(a)(2): Aerial lifts were "field modified" for uses other than those intended by the manufacturer, the modification had not been certified in writing by the manufacturer or by any other equivalent entity. Modification did not conform with all applicable provisions of ANSI A92.2-1969: Worksite located at 524 NW Englewood Rd., Kansas City, Missouri- Employees were exposed to a fall hazard in that the employer had modified the lift in order to removed materials from the roof. The modifications had not been approved by an approval entity and the employees had overloaded the lift which could cause the employees to fall from elevation.

29 CFR 1926.453(b)(2)(vi): Boom and basket load limits specified by the manufacturer were exceeded: Worksite located at 524 NW Englewood Dr., Kansas City, Missouri- Employees were exposed to fall hazards in that the load limit was exceeded which could cause the boom to collapse or catapult an employee out of the lift. One employee had two lanyards connected to one another which, if he fell would allow the employee to fall more than 12' and the force of this fall would also exceed the load limit.
