1926.20(b)(2)  The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a. On or about October 13, 2020, at the above addressed jobsite, frequent and regular inspections of the jobsite were not performed by a competent person. An employee observing a sub-contractor performing residential roofing operations on a roof, eighteen (18) feet three (3) inches above the ground, was not able to identify fall hazards, thereby exposing the sub contractor employees to fall hazards.

29 CFR  1926.503(c):The employer did not retrain affected employees  who already had been trained but demonstrated inadequate  understanding and skill required by paragraph (a) of this section:  a. On or about October 13, 2020, at the above addressed jobsite, an employee required to monitor subcontractor employee and conduct inspections was not retrained on fall protection when the employee demonstrated the lack of understanding of fall protection requirements, thereby exposing the employee to fall hazards.
