29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: West side of the commercial construction site: On or about October 15, 2020, and at times prior thereto, employees were exposed to a fall hazard while using a Werner extension ladder without proper training.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways: West side of the commercial construction site: On or about October 15, 2020, and at times prior thereto, employees were exposed to a fall hazard while using a Werner extension ladder which was leaned against the wall of a building, and not placed against the barrier wall being accessed.
