29 CFR  OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that a employee was exposed to being struck by the excavator bucket:  (a) Excavation site: On or about 10/19/20 , and at times prior , employees were exposed to struck by hazards while working in and around the Cat-308E2 excavator. The employer failed to properly verify the quick coupler attachments were engaged after changing attachments.
