In the trusses of 4th floor of Building 6000, located at 7738 AC Skinner Parkway, Jacksonville FL: On or about October 20, 2020, the employer exposed subcontractor's employees to a 13 foot fall hazards, in that a a ladder was not provided to employees accessing the work area on top of trusses.
