29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. On or about October 20, 2020, at the south side of the residential home, an employee applying silicone on the exterior wall decoration was standing and walking on a 7:12 pitch roof, exposing to a 9 feet fall hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a. On or about October 20, 2020, at the south side of the residential building, the side rails of the Werner extension ladder being used to get access to the roof extended about 20 inches to the upper landing surface, exposing employees to a 9 feet fall hazard.
