On or about October 20, 2020 at the job site located at 516 Broughton Drive Sheboygan, WI, the employer did not instruct employees, operating equipment to demolish a building that contained lead based paint, how to recognize and avoid unsafe conditions and the regulations applicable to their work environment to control or eliminate lead exposure.

29 CFR 1926.62(h)(1):"All surfaces shall be maintained as free as practicable of accumulations of lead". a. On or about October 20, 2020 at the job site at 516 Broughton Drive Sheboygan, WI, the employer did not maintain all surfaces as free as practicable of lead accumulations. An employee operating an excavator was exposed to levels of 458.19 micrograms/ft2 as measured with a wipe sample taken on the right joystick of the Komatsu excavator. b. On or about October 20, 2020 at the job site at 516 Broughton Drive Sheboygan, WI, the employer did not maintain all surfaces as free as practicable of lead accumulations. Employees were exposed to levels of 942.3 micrograms/ft2 as measured with a wipe sample taken on the left side joystick of the Caterpillar skid steer loader.

29 CFR 1926.62(d)(1)(i):Each employer who has a workplace or operation covered by this standard shall initially determine if any employee may be exposed to lead at or above the action level. On or about October 20, 2020 at the job site located at 516 Broughton Drive Sheboygan, WI, the employer did not determine if any employee may be exposed to lead at or above the action level. Two machine operators had not been monitored for lead exposure prior to demolishing a former armory that had been identified as containing lead based paint.

On or about October 20, 2020 at the job site located at 516 Broughton Drive Sheboygan, WI, the employer did not collect personal samples representative of a full shift for the operators demolishing a former armory that had been identified as containing lead based paint.
