29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems: On or about October 20, 2020 an employee was not protected from a fall while hoisting/ lowering roofing equipment to a lower level, exposing the employee to a fall of approximately 10 feet. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about October 20, 2020 employees engaged in roofing operations were not protected from falling through roof holes, exposing employees to a fall of over 6 feet. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about October 20, 2020 employees engaged in roofing operations from a flat roof were not protected from a fall, exposing employees to a fall of 23.8 feet. Nick Wyka dba Addison Construction and Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1304231, citation number 1, item number 1 and was affirmed as a final order on May 22, 2018, with respect to a workplace located at 443 W Lake St. Addison, IL 60101.
