29 CFR 1910.28(b)(3)(i)(A):The employer did not ensure that each employee on a walking-working surface was protected from falling into any hole or pit that was 4 feet (1.2 m) or more above a lower level by a cover or covers: (a) Jiffy Lube Shop, Bay 1: On or about, October 7th, 2020, the employer failed to protect employees from fall hazard of approximately 6 feet when the net meant to cover the pit of bay 2 had several attachment bolts that were missing. This created gaps between the net and the rail and weakened the strength of the net to properly protect employees.
