29 CFR  1910.107(b)(9):A clear space of not less than 3 feet on all sides was not kept free from storage or combustible construction:  (a) Facilities: Paint Booth: The employer had a wooden table on the right side of the paint booth with storage of various items such as WD-40, paper towels, sponges, buckets, and tools.  Employees use category 3 flammable paints to paint various work pieces.  The employees are potentially exposed to a fire hazard.

29 CFR  1904.29(b)(6): The employer did not enter "privacy case" in the space normally used for the  employee's name on the OSHA 300 form or equivalent.   (a) On or about July 13, 2020 and times prior to, the employer listed the names of employees that sustained needlestick injuries and cuts from sharp objects that were potentially contaminated with another person's blood or other potentially infectious material.

29 CFR  1910.134(e)(6)(i):The employer did not obtain from the physician or other licensed health care professional (PLHCP)  a written recommendation regarding the employee's ability to use the respirator:  (a) Facilities: Paint Booth: Employees wore tight-fitting half-mask elastomeric respirators while conducting painting operations.  The employer did not have a written recommendation from a physician or PLHCP for these employees to wear the respirators.  This condition potentially exposes the employees to exacerbated health condition(s).    Abatement Note:  The recommendation shall provide only the following information: (1)  Any limitations on respirator use related to the medical condition of the employee, or relating to the workplace condition in which the respirator will be used, including whether or not the employee is medically able to use the respirator;  (2)  The need, if any, for follow-up medical evaluations; and, (3)  A statement that the PLHCP has provided the employee with a copy of the PLHCP's written recommendation.
