29 CFR  1926.152(g)(11): Each service or fueling area shall be provided with at least one fire extinguisher having a rating of not less than 20-B:C located so that an extinguisher will be within 75 feet of each pump, dispenser, underground fill pipe opening, and lubrication or service area.   On or about October 2, 2020 and times prior thereto, the employer did not have any fire extinguishers at the fueling area where the 500 gallon above-ground (fuel tank) dispensing unit was used; exposing employees of fire hazards.

29 CFR  1926.20(b)(2):Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On or about October 2, 2020 and at times prior thereto, the employer failed to conduct frequent jobsite safety inspections to identify and provide a workplace were any employee, worker, laborer, or mechanic employed in the performance of the contract work was free from recognized hazards such as but not limited to:  a)  Workers worked from unguarded balconies without fall protection.  Workers worked from balconies without fall protection while performing framing activity; workers exposed to a fall hazard of up to 20 feet to the ground below.   b)  Workers worked from aerial lifts with out being tied off; exposing the them to fall hazards of up to 30 feet to the ground below.     c)  Workers worked on top floor of a 3rd story apartment complex working near the edge performing framing activity without fall protection, were exposed to a fall hazard of up to 30 feet to the ground below.     The (Multicon Services, LLC) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(2),  which was contained in OSHA inspection number  1380033, citation number 1,  item number 1  and was affirmed as a final order on 4/25/2019, with respect to a workplace located at 400 S. Oak St., Roanoke, TX  76262.
