29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.   On or about October 21, 2020, and at times prior thereto, employees operating Hitachi nail guns while performing decking activities were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.  a) An employee was on the front side of building 6 performing decking work without safety glasses.   b) An employee was on the rear side of building 6 performing decking work without safety glasses.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated  On or about October 21, 2020, and at times prior thereto, employees were decking while inside an elevated aerial lift:  a) An employee was on the front side of building 6 not wearing fall protection and was not attached to the boom or basket, exposing the employee to being ejected and unprotected fall hazards of approximately 10-feet to the ground below.   b) An employee was on the front side of building 5 not wearing fall protection and was not attached to the boom or basket, exposing the employee to being ejected and unprotected fall hazards of approximately 10-feet to the ground below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about July 13, 2020, and at time prior thereto, employees engaged in framing activities, were not provided fall protection exposing them to unprotected fall hazards of approximately 10 feet.  a) An employee was on the front side of building 6 working on a surface with unprotected side or edge.   b) An employee was on the rear side of building 6 working on a surface with unprotected side or edge.
