29 CFR 1926.502(d)(1):Personal fall arrest system connectors were not drop forged, pressed or formed steel, or made of equivalent materials: a) On or about October 13, 2020 at 1900 Stanford Place Ct., Auburn, Al: The employer failed to ensure an employee pulling up and installing exterior trim boards had his rope connected to the anchor with a snap hook in that his rope was tied with knot to the D-ring of his harness.

29 CFR 1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person: a) On or about October 13, 2020 at 1900 Stanford Place Ct., Auburn, Al: The employer failed to ensure an employee pulling up exterior trim boards and installing them was properly anchored in that he wrapped his lanyard around a 2x6 truss board, which is not designed to support a 5,000 pound shock load during a fall, allowing employees to fall 33 feet, 5 5/8 inch to the ground below. b) On or about October 13, 2020 at 1900 Stanford Place Ct., Auburn, Al: The employer failed to ensure an employee pulling up exterior trim boards and installing them was properly anchored in that he wrapped his rope around a 2x6 truss board, which is not designed to support a 5,000 pound shock load during a fall, allowing employees to fall 33 feet, 5 5/8 inch to the ground below.
