29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.  a) Miguel Dominguez, at a jobsite located at 768 Wharncliff, Lot WEL138, Pickerington, Ohio 43062: On and/or preceding October 20, 2020, the employer did not ensure extension cords were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Roofers were using a yellow extension cord with missing insulation along its length and a missing ground prong, which exposed the employees to a 123 volt electrical contact hazard.

The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.  a) Miguel Dominguez, at a jobsite located at 768 Wharncliff, Lot WEL138, Pickerington, Ohio 43062: On and/or preceding October 20, 2020, the employer did not ensure employees who used extension cords were protected from electrical contact hazards. Roofers were using a yellow extension cord with the ground prong missing from the male end of a cord, which exposed the employees to a 123 volt electrical contact hazard.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  a) Miguel Dominguez, at a jobsite located at Lot RVO425, 544 Randall Road, Delaware, Ohio 43015: On and/or preceding October 20, 2020, the employer did not ensure portable ladders that were used for access to an upper landing surface were extended at least 3 feet (.9 m) above the upper landing surface. Employees were using extension ladders that did not extend at least 3 feet above the edge of the roof, thereby exposed to fall hazards of up to 20 feet.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  a) Miguel Dominguez, at a jobsite located at 768 Wharncliff, Lot WEL138, Pickerington, Ohio 43062: On and/or preceding October 20, 2020, the employer did not ensure employees were adequately protected from falls while engaged in residential construction activities. Employees was installing roofing materials with non-safety rated fall protection equipment, thereby exposed to fall hazards of up to 20 feet.  Miguel Dominguez was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1481682, citation number 2, item number 2 and was affirmed as a final order on September 10, 2020, with respect to a workplace located at 768 Wharncliff, Lot WEL138, Pickerington, Ohio 43062.  Miguel Dominguez was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1418200, citation number 2, item number 1, and was affirmed as a final order on August 23, 2019, with respect to a workplace located at 137 Thrush Circle, Pickerington, Ohio, 43147  Miguel Dominguez was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1261589, citation number 1, item number 1, and was affirmed as a final order on 10/10/2017, with respect to a workplace located at 6400 Brandon Drive, Lewis Center, Ohio.
