29 CFR 1926.651(j)(2):Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: a)a) 6804 S. Ensenada St, Aurora, CO, 80116: On or about October 6, 2020, and at times prior, Ibarra's Excavation Services, LLC, as a creating and correcting employer, did not ensure that employees of AML Caisson and Speer Forming were protected from suffocation or crushing hazards while working in an excavation next to materials which were not protected from falling or rolling back into the excavation. Excavated materials were located less than two feet from the edge of the excavation and were observed to be falling back into the excavation. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): a) 6804 S. Ensenada St, Aurora, CO, 80016: On or about October 6, 2020, and at times prior, Ibarra's Excavation Services, LLC, as a creating and correcting employer, did not ensure that employees of Speer Forming and AML Caisson LLC were protected from cave-ins while working in an excavation greater than five feet deep. The excavation did not have an adequate protective system to prevent cave-ins. Cave-ins were observed occurring in several places around the excavation. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
