On or about October 21, 2020, the employer did not ensure employees were protected from amputation hazards while operating the Blakeslee Commercial Food Mixer (Serial # 2-5798-1AAA). An employee sustained a right hand degloving while pushing blocks of cheese through the cheese grating attachment after the glove became caught in the blade.
