29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about October 22, 2020, at a work-site located next to 9987 Rosalee Ln, Strongsville, Ohio 44136; during the installation of framing material, the employer did not ensure employees working on the 1st floor feeding material to the framers above was protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 11.3 feet.

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about October 22, 2020, during the installation of framing material at the job site located at 9987 Rosalee Ln, Strongsville, Ohio 44136; the employer did not ensure employees working on the second floor of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing framing material with air driven nail guns.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about October 22, 2020, during the installation of framing material at a job site located at 9987 Rosalee Ln, Strongsville, Ohio 44136; the employer did not ensure employees working on the second floor of a single family home were protected from a fall of approximately 11.3 feet by utilizing a fall protection system.
