29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.   a) At the jobsite, on the scaffold, on or about 10/22/20:  Workers access and egressed the scaffold platform by climbing over the pick and pick bracket support.

29 CFR  1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).Note:  Specify distances as indicated in table in paragraph (f)(6) of this section.   a) At the jobsite, on or about 10/22/20: Employees were working on an aluminum ladder-jack scaffold where that scaffold was within 6 inches of a live power supply line.

29 CFR  1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.  a) At the worksite, on the scaffold, on or about 10/22/20: Roofers worked on a ladder jack scaffold, where the pick surface was approximately 19 feet from ground and where no fall protection was provided.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)  NOTE TO CSHO: USE THIS OPTION ONLY IN CASES WHERE THE INFEASIBILITY/GREATER HAZARD EXCEPTION IS NOT IN ISSUE.   a) At the jobsite, on or about 10/22/20:  Workers were installing roofing material on a 8:12 pitch roof, with a ground to eave height of 24 feet, without fall protection of any kind.
