29 CFR  1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a.  On or about October 22, 2020, on the north side of the building, employees were working from an aerial lift without the use of harness and lanyard attached to the boom or basket, exposed to a 30 feet fall hazard.  b.  On or about October 22, 2020, on the east side of the building, employees were working from an aerial lift without the use of harness and lanyard attached to the boom or basket, exposed to a 20 feet fall hazard.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a.  On or about October 22, 2020, on the east side of the building, an employee was installing siding from a flat roof without the use of a fall protection system, exposing to a 15.5 feet fall hazard.
