29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  On 10/13/2020, at the Morgan Landing subdivision, lots 27-33, Buford, Ga., the employer was not performing inspections of the job site so as to ensure employees were working safely. Employees were exposed to fall hazards greater than 10' and working from improperly built scaffolds at the site.

29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  On 10/13/20, at the Morgan Landing subdivision, Lot 41, Buford Ga, employees worked from scaffolds that did not have the working level of the scaffold fully decked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.   On 10/13/20, at the Morgan Landing subdivision, Lot 41, Buford Ga, employees working from scaffolds were accessing working levels by climbing the scaffold members themselves or accessing working levels from 2nd story roofs rather than using a ladder to access working levels.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.  On 10/13/20, at the Morgan Landing subdivision, Lot 41, Buford Ga, employees worked from scaffolds that had not been inspected by a competent person. Deficiencies included not having the working level of the scaffold fully decked and not having access to working levels.

On 10/13/2020, at the Morgan Landing subdivision, Buford, Ga., an employee was exposed to fall hazards greater than 10' as the employee accessed porch roof levels to then access a scaffold without fall protection systems.
