29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about October 21, 2020, the employer did not ensure that employees were in use of head protection while performing trenching work activities from inside of a trench with spoil piles located on the edge of the trench.  This exposed the employees to a struck-by hazards.
