29 CFR  1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions:  a) On or about October 22, 2020, and at times prior, workers were exposed to electrical shock hazards while operating a Sunrise ironworker model IW-66K, serial number 13047931, while the foot pedal conductors entering the cabinet were not protected from abrasions. 29 CFR  1910.334(a)(2)(ii):There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service:  a) On or about October 22, 2020, and at times prior, workers were exposed to electrical shock hazards while operating a handheld Bosch grinder model GSW 10-45 and another Bosch grinder with cord damage while the tool remained energized and in service.  b)  On or about October 22, 2020, and at times prior, workers were exposed to electrical shock hazards while operating a handheld Bosch grinder connected to a flexible cord set (extension cord) with outer cover damage near the cord end.  Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR  1910.29(f)(4):Openings in a stair rail system exceeded 19 inches at its least dimension:  1) On or about October 22, 2020, and at times prior, workers were exposed to fall hazards at the stair set between the main floor and second level where opening between the stair and top rail was 32 inches without the installation of a mid rail.  Abatement Note: The employer corrected during inspection, the Certification of Corrective Action Worksheet is not required.
