29 CFR  1910.334(a)(2)(ii):There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service:  a) On or about October 22, 2020, and at times prior, workers were exposed to electrical shock hazards while operating a handheld Bosch grinder model GSW 10-45 and another Bosch grinder with cord damage while the tool remained energized and in service.  b)  On or about October 22, 2020, and at times prior, workers were exposed to electrical shock hazards while operating a handheld Bosch grinder connected to a flexible cord set (extension cord) with outer cover damage near the cord end.  Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).
