29 CFR 1910.23(c)(7): Ladders placed in locations such as passageways, doorways, or driveways where they can be displaced by other activities or traffic were not secured to prevent displacement or guarded by a temporary barricade, such as a row of traffic cones or caution tape, to keep activities away from the ladder:  a. On October 21, 2020, in the kitchen, the  ladder used in front of the walk-in freezer doorway was not secured to prevent the ladder from being displaced, exposing an employee to an 7 feet fall hazard.

29 CFR 1910.23(c)(8): The employer did not ensure that the top cap and the top step of a step ladder were not used as steps:  a. On October 21, 2020, in the kitchen, the green ladder used to access the crawlspace above the walk-in freezer did not extend high not to allow the safe transition from the ladder to the next level.

On October 21, 2020, in the kitchen, on top of the walk-in freezer, the parapet wall was not adequate height to prevent employees from a 7 feet fall hazard.
