29 CFR 1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about October 21, 2020, the employer failed to ensure employees performing decking activity on a low sloped roof had fall protection, exposing employees to fall hazards of 18 feet to the ground below.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. On or about October 21, 2020, the employer failed to provide employees a safe means of accessing/egressing the roof, exposing employees to fall hazards.
