The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  A) New building construction - 957 Stuyvesant Avenue, Union, NJ 07083  The employer did not certify that the operator of the CAT Telehandler TL1255D was trained as required by the standard.  Exposure occurred on or about 10/23/2020.

29 CFR 1926.451(b)(1)(i): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and the guardrail supports such that space between adjacent units, and the space between the platform and the uprights.  A) New building construction - 957 Stuyvesant Avenue, Union, NJ 07083  The employer did not ensure that each working level of a scaffold was fully planked or decked between the front uprights and the guardrail supports, exposing employees working on the platform to fall hazards.  Exposure occurred on or about 10/23/2020.

Exposure occurred on or about 10/23/2020.

29 CFR  1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold shall be protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity).  A)    New building construction - 957 Stuyvesant Avenue, Union, NJ 07083  The employer did not ensured the tubular welded frame scaffold had guardrails on all open sides and ends of the scaffold exposing employees to falls of approximately 30 feet.  Exposure occurred on or about 10/23/2020.
