29 CFR 1926.34(a): Building or structure exits were not so arranged and maintained as to provide free and unobstructed egress from all parts of the building or structure at all times when it was occupied: a) Downstairs - On or about October 20, 2020, employees performed electrical work removing and installing new wiring for a series of circuit breakers in a 120 volt sub-control panel. The exit was blocked with a waste bin, preventing the exit door from opening and was not arranged to provide free and unobstructed egress with the building occupied, exposing employees to fire hazards.

29 CFR 1926.34(b): Exits and access to exits were not marked by a readily visible sign: a) Downstairs - On or about October 20, 2020, employees performed electrical work removing and re-installing new wiring for a series of circuit breakers into a 120 volt sub-control panel. The exit for employee egress was observed without signage posted for visibility, exposing employees to fire hazards.

29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged: a) Downstairs - On or about October 17, 2021, an employee removed and installed new wiring for a series of circuit breakers into a 120 volt sub-control panel. The employee had de-energized the main control panel outside of the building without the use of locks or tags, exposing the employee to electrical hazards.
