29 CFR  1910.38(b): An emergency action plan was not in writing, kept in the workplace, and available to employees for review:  On or about October 26, 2020, the employer did not maintain an emergency action plan for employees. The employer does not cover the minimal elements of an emergency action plan with employees including: procedures for reporting a fire or other emergency; procedures for emergency evacuation, including type of evacuation and exit route assignments; procedures to be followed by employees who remain to operate critical plant operations before they evacuate; procedures to account for all employees after evacuation; procedures to be followed by employees performing rescue or medical duties; and the name or job title of every employee who may be contacted by employees who need more information about the plan or an explanation of their duties under the plan.

29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about October 26, 2020, the employer failed to establish an energy control program for employees performing service and maintenance on machines and equipment. The employer had trim saws, sidewinders or pallet break down saws, pop up saws, knotchers, bandsaws, and gang rip saws with multiple energy sources including air and electric. Employees were required to perform service and maintenance with no written procedures, training and/or periodic inspections.
