29 CFR  1910.132(a):Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition.  On or about October 27, 2020, and at times prior thereto, TSF Sportswear did not provide protective equipment for eyes, face, and specific protective clothing for employees servicing forklift batteries. An employee servicing batteries was exposed to injuries from battery acid when filling power industrial truck batteries.

29 CFR  1910.157(c)(1):Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  On or about October 27, 2020, and at times prior thereto, TSF Sportswear did not ensure that all portable fire extinguishers located within the warehouse were identified so that they were readily accessible to employees. 29 CFR  1910.176(b):Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.  On or about October 27, 2020, and at times prior thereto, TSF Sportswear did not ensure materials stored on storage racks were placed on the racks properly, stable and/or secured from falling from the storage racks. Employees were potentially exposed to struck-by hazards.  1. Warehouse Rack Row C/D, upper levels boxes not securely placed. 2. Warehouse Rack Row D 14H1,  boxes not securely placed. 3. 29 CFR  1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage:  On or about October 27, 2020, and at times prior thereto, TSF Sportswear did not ensure electric equipment within the warehouse was protected from exposure to physical damage, such as but not limited to, barricaded to prevent damage from power industrial trucks operated daily in the warehouse.  Employees were exposed to electrical shock hazards.

29 CFR  1910.305(e)(1):Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed to prevent moisture or water from entering and accumulating within the enclosures:  On or about October 27, 2020, and at times prior thereto, TSF Sportswear did not ensure electrical cabinets with circuit breakers and switches used to energize fans, battery chargers and other electrical equipment were protected and not located in areas where wet conditions existed, such as but not limited to, a water source used to service forklift batteries.  Employees were potentially exposed to electrical shock hazards.
