29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) 247 East Fulton ST.  Gloversville, NY-On or about October 23, 2020, an employee working from jacks and planks (slide guards) conducting roofing operations was exposed to a fall hazard and had no method of fall arrest or fall restraint protecting the employee from falling tot he next lower level.  b) 247 East Fulton ST.  Gloversville, NY-On or about October 23, 2020, two employees were conducting roofing operations and working from a first level roof, with no method of fall arrest or fall restraint protecting them from falling to the next lower level. .

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) 247 East Fulton Street.  Gloversville, NY-On or about October 23, 2020, employees were exposed to a fall hazard while conducting roofing activities on a two story residential home, were not provided training to identify hazards and be able to take corrective action to abate the hazard the employees were exposed to.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) 247 East Fulton St. Gloversville, NY-On or about October 23, 2020, employees conducting roofing operations were exposed to a fall hazard from two extension ladders that did not extend three feet above the upper landing surface resulting in unsafe access and creating a fall hazard.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause them to lose balance and fall:  a) 247 East Fulton St. Gloversville, NY-On or about October 23, 2020, An employee was exposed to a fall hazard while climbing an extension ladder. The employee was carrying bulky roofing material in one hand eliminating his ability to maintain three points of contact exposing the employee to a fall hazard.
