29 CFR  1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE).   a) Jobsite located at 151 N Independence Mall E, Philadelphia, PA: The employer failed to update their hazard assessment for the carpentry shop and assess the hazards of a Powermatic 2000 hand-fed table saw, which employees used to cut materials such as wood, plexiglass, etc.  An employee operating the table saw was struck by a piece of wood that kicked back and was not protected with personal protective equipment (PPE) such as a face shield.   Condition noted on/or about October 24, 2020.

29 CFR  1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use PPE. Each such employee shall be trained to know at least the following: (i)-(v):   a) Jobsite located at 151 N Independence Mall E, Philadelphia, PA: The employer required the use of PPE, such as safety glasses and safety shoes, but failed to provide PPE training for employees exposed to the hazards of a Powermatic 2000 hand-fed table saw, which employees used to cut materials such as wood, plexiglass, etc.    Condition noted on/or about October 24, 2020.

29 CFR  1910.213(d)(1): Each circular crosscut table saw was not guarded by a hood which shall meet all the requirements of paragraph (c)(1) of this section for hoods for circular ripsaws.  a) Jobsite located at 151 N Independence Mall E, Philadelphia, PA: Employees were exposed to amputation hazards while operating a Powermatic 2000 crosscut table saw in that the hood guard was not installed prior to use.  Condition noted on/or about October 24, 2020.

29 CFR  1910.213(c)(3): Each hand-fed circular ripsaw was not provided with nonkickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator. They shall be designed to provide adequate holding power for all the thicknesses of materials being cut.   a) Jobsite located at 151 N Independence Mall E, Philadelphia, PA: Employees were exposed to struck-by hazards while operating a Powermatic 2000 hand-fed table saw in that the anti-kickback pawl was not installed on the table saw prior to use.  Condition noted on/or about October 24, 2020.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  a) Jobsite located at 151 N Independence Mall E, Philadelphia, PA: The employer did not have a written certification of hazard assessment in accordance with this standard and did not effectively assess the hazards present in the workplace. The employer failed to assess in writing the amputation, laceration, struck-by, and caught in-between hazards in the carpentry shop in that a hazard assessment performed in 2004 was not updated after a Powermatic 2000 hand-fed table saw had been placed into service on or about CY2012.
