29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about October 14, 2020 and times prior at 3004 Alumni Lane, Auburn, Al: The employer failed to ensure employees installing sheathing on a 14:12 pitch roof used fall protection which exposed them to the hazards associated with falls of 9"10" to the ground below. b) On or about October 14, 2020 and times prior at 3004 Alumni Lane, Auburn, Al: The employer failed to ensure employees installing decking above the garage used fall protection which exposed them to the hazards associated with falls of 10'4" to the concrete floor. c) On or about October 14, 2020 and times prior at 3004 Alumni Lane, Auburn, Al: The employer failed to ensure employees installing plywood siding for the garage used fall protection which exposed them to the hazards associated with falls of 9'10" to the ground below.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about October 14, 2020 and times prior at 3004 Alumni Lane, Auburn, Al: The employer failed to ensure the railings of the extension ladder extended at least 3 feet above the landing area for employees installing siding material on the outside of the garage which exposed them to the hazards associated with falls of 9'10" to the ground.
