29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative.   On or about October 15, 2020 and at times prior there to, the employer did not establish an energy control program to isolate all energy sources to safely work on machinery.  This exposed employees to amputation and caught-in hazards.

29 CFR  1910.147(c)(7)(i)(A):  Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.   On or about October 15, 2021, the employer did not ensure the authorized employee was trained to lock out the RE20 thermoforming packaging machine to safely perform maintenance.  This exposed the employee to amputation, caught in, and electrical hazards.

On or about October 15, 2020 the employer did not ensure that a lock was affixed to all energy sources on the RE20 thermoforming packaging machine.  This exposed the employee to an amputation and caught-in hazard.
