29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about September 29, 2020, an employee received an amputation as a result of a work-related incident, and it was not reported to OSHA within 24 hours. The injury was originally categorized improperly and after internal review it was identified as an amputation of the fingertip, the amputation was reported to OSHA on October 5, 2020.
