29 CFR OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. (a) On or about 10/20/2020 and thereafter - 15191 Highway 243, Russellville, AL 35654, employees were exposed to crushed-by hazards when a 600 pound housing was not secured to prevent overturning and a 9 inch grinder was used to remove weld splatter from the surface of the housing.

29 CFR 1910.179(g)(1)(v): Pendant control box(es) were not clearly marked for identification of functions: (a) On or about 10/20/2020 and thereafter- 15191 Highway 243, Russellville, AL 35654, employees operated a 15 ton overhead crane using an eight button pendent to move 600 pound metal housings through the warehouse and the pendant used to operate the crane was not clearly marked.
