29 CFR 1910.106(b)(6):In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: SHOPPETTE: Hand tools capable of creating sparks were used to loosen and open valves during LPG tank refills, exposing employees to explosion hazards.

29 CFR 1910.178(f)(2): The storage and handling of liquefied petroleum gas fuel was not in accordance with NFPA Storage and Handling of Liquefied Petroleum Gases (NFPA No. 58-1969), which is incorporated by reference as specified in Sec. 1910.6. SHOPPETTE: During LPG tank refills face shields and arm coverings were not worn exposing employees to cold burns.
