29 CFR  1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  a) Patterson Middle Spice Mixer Machine, SN: CA9777, Company number IS/ID#MXR-MIX01-04:  On or about October 27, 2020 ,  the employer exposed employees to potential hazardous energies in that employees were required to clean the spice mixer machine  without first affixing a lockout device on the machines isolating device to secure the machine from the energy source.
