On or�about November 5, 2020,�an employee in the blending area was exposed to respiratory hazards while wearing a 3M brand half-facepiece respirator and a 3M brand full-facepiece respirator without having been fit tested before initial use. b. On or�about November 5, 2020,�employees in the blending area were exposed to respiratory hazards while wearing 3M brand half-facepiece respirators without having been fit tested annually.

29 CFR 1910.134(g)(1)(i)(A):Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: On or about November 5, 2020, an employee was required to wear a tight fitting 3M brand half-face respirator without having their facial hair trimmed or shaven so that the facial hair did not interfere with the face mask sealing surface.
