Crane Street Norton, Kansas 67654:  On or about October 27, 2020, and at times prior thereto and thereafter, the employer had not conducted an assessment to determine if hazards such as SARS-CoV-2, the virus that causes COVID-19, were present that necessitated the use of PPE in order to protect employees who had contact with suspected or confirmed COVID-19 residents.

Crane Street Norton, Kansas 67654:  On or about October 27, 2020, and at times prior thereto and thereafter, employees providing nursing services were not provided with respirators for protection against aerosolized SARS-CoV-2 virus. At a minimum, a N95 NIOSH-certified filtering facepiece respirator is necessary to protect employees from aerosolized SARS-CoV-2 virus.

Crane Street Norton, Kansas 67654:  The employer did not develop and implement a written respiratory protection program with worksite-specific procedures for respirator use that included all provisions in 29 CFR 1910.134(c)(1)(i) - (ix).     a)	On or about October 12  through October 17, 2020, the employer required employees to wear N95 face piece respirators while assisting residents with respiratory treatments   b)	 On or about October 27, 2020 , and at times prior thereto and thereafter, employees were required to wear respirators labeled "KN95" throughout their shifts while exposed to suspected or confirmed positive COVID-19 residents and staff .

Crane Street Norton, Kansas 67654:  On or about October 27, 2020, and at times prior thereto and thereafter, the employer did not provide NIOSH-certified respirators to employees who were required to care for suspected and confirmed positive COVID-19 residents. The employer provided and required employees to wear respirators labeled "KN95" with ear loops. However, the KN95 respirators did not contain certification markings indicating they were certified under an equivalent international standard.
