On or about October 30, 2020, at the above addressed jobsite, employees installing holiday decorative lighting on a residential home, seventeen (17) feet six (6) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.

On or about October 30, 2020, at the above addressed jobsite, employees installing holiday decorative lighting on a residential home were not provided training on how to minimize fall hazards, thereby exposing employees to the hazards associated with falls.
