29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about November 2, 2020, employees were exposed to a struck by hazard while performing roofing activities on a residential home located at 8877 Sherry, Macedonia, Ohio 44056. Employees were working on the roof with tools and materials that could have fallen on lower employees, the ground employees were not wearing protective helmets.

The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.
