29 CFR  1926.20(b)(1):The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a) Alan Vylor, DBA: LB Construction, on or about October 30th, at the location of 36539 Amalfi Rd., Sublot 143, North Ridgeville, OH, 44039: Employees performing framing work on a new residential structure were exposed to fall hazards of approximately 16 feet to the ground below and struck by hazards while using pneumatic nail guns without the benefit of accident prevention programs to ensure appropriate measures were taken to mitigate risks.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Alan Vylor, DBA: LB Construction, on or about October 30th, at the location of 36539 Amalfi Rd., Sublot 143, North Ridgeville, OH, 44039: Employees performing framing work on a new residential structure were exposed to fall hazards of approximately 16 feet to the ground below without the benefit of fall protection training to ensure that employees were informed on the appropriate measures to implement to prevent falls.
