29 CFR  1926.34(a):Building or structure exits were not so arranged and maintained as to provide free and unobstructed egress from all parts of the building or structure at all times when it was occupied:  a) Christian's Roofing LLC, on or about October 30th, 2020, at the location of  Homesite 84, Cedar Crest Dr., Elyria, OH, 44035:  Employees were performing roofing work on top of a new residential structure exposed to a fall hazard of approximately 12 feet with a portable ladder as the only access point.  The ladder was removed for another employee to use elsewhere, giving the employees no safe exit route in the event of an emergency such as a medical emergency, injury, fire, or other hazardous circumstances.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Christian's Roofing LLC, on or about October 30th, 2020, at the location of  Homesite 84, Cedar Crest Dr., Elyria, OH, 44035:  Employees were performing roofing work on top of a new residential structure while exposed to fall hazards of approximately 12 feet to the ground below without the benefit of fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Christian's Roofing LLC, on or about October 30th, 2020, at the location of  Homesite 84, Cedar Crest Dr., Elyria, OH, 44035:  Employees were using a portable extension ladder to access a roof to performing roofing work on top of a new residential structure.  The extension ladder did extend at least 3 feet above the roof, exposing employees to potential fall hazards of approximately 12 feet to the ground below while accessing the roof.
