Site - On or about October 22, 2020, employees were exposed to fall hazards while climbing up to and from the deck and onto the ladder, where there was a break of elevation greater than 19 inches.  Instance b. Site - On or about October 22, 2020, employees working from the scaffolding system were exposed to fall hazards while climbing up to and from the roof area, where there was a break of elevation greater than 19 inches.
