29 CFR  1926.1051(a):Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  Instance a. Site - On or about October 22, 2020, employees were exposed to fall hazards while climbing up to and from the deck and onto the ladder, where there was a break of elevation greater than 19 inches.  Instance b. Site - On or about October 22, 2020, employees working from the scaffolding system were exposed to fall hazards while climbing up to and from the roof area, where there was a break of elevation greater than 19 inches.
