29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Osman "Alex" Inestrosa dba Lifetime Contractor Corp., does not ensure that employees use appropriate head protection where employees were exposed to overhead hazards. This violation was most recently observed at 90 Mola Drive, Elmwood Park, NJ, at instance (a): Employees engaged in car port garage roofing activities were exposed to an overhead impact hazard while positioned at ground level beneath an active roofing operation, as the employer failed to ensure the use of head protection was utilized. Condition noted on or about 10/24/2020. To abate this violation the employer must ensure that its employees performing roofing activities are protected from head injury and that no employee is allowed to be exposed to head injuries without wearing proper head protection.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents :   Osman "Alex" Inestrosa dba Lifetime Contractor Corp., does not ensure that employees use appropriate eye protection when exposed to eye injury hazards, including but not limited to when using tools such as a circular electric saw, a pneumatic nail gun and a hammer. This violation was most recently observed at 90 Mola Drive, Elmwood Park, NJ, at instance (a): Employees engaged  in a car port  garage roofing operation using  pneumatic guns were exposed to struck-by hazards with the potential to cause eye injuries or loss.  The employer did not ensure the use of appropriate eye protection for these employees, on or about 10/24/2020. To abate this violation the employer must ensure that its employees performing roofing activities are protected from eye injury and that no employee is allowed to be exposed to eye injuries without wearing proper eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   Osman "Alex" Inestrosa dba Lifetime Contractor Corp, does not ensure that employees use adequate fall protection when working at elevated heights.. This violation was most recently observed at 90 Mola Drive, Elmwood Park, NJ, at instance (a): Employees engaged in roofing work were severely injured when a car port garage collapse of approximately 12 feet to the concrete below. The employer did not ensure the use of adequate fall protection for these employees, on or about 10/24/2020. To abate this violation the employer must ensure that its employees performing roofing activities re protected from fall hazards and that no employee is allowed to be exposed to fall hazards without utilizing adequate fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Osman "Alex" Inestrosa dba Lifetime Contractor Corp., does not ensure that employees use appropriate head protection where employees were exposed to overhead hazards. This violation was most recently observed at 90 Mola Drive, Elmwood Park, NJ, at instance (a): Employees engaged in car port garage roofing activities were exposed to were exposed to fall hazards of approximately 12ft to the concrete below when an extension ladder used to access the roof had side rails that did not extend 3 feet above the upper landing surface. Condition noted on or about 10/24/2020. To abate this violation the employer must ensure that its employees performing roofing activities extend the ladder at least 3 feet above the upper landing.
